Draft → In Progress → Submitted → Approved

Creating Reports
Step 1: Start Report
- Go to Reports → DSHS Reports
- Click + New Report
- Select report type
Step 2: Select Client
- Search client name
- System shows:
- Authorization period
- Previous reports
- Due dates
Step 3: Set Period

Adding Content
Goals Section
- Click + Add Goal
- Enter:
- Goal description
- Target date
- Progress notes
- Mark status: Active, Achieved, Modified
Progress Notes
Document progress:
- Summarize period
- Note achievements
- Describe challenges
- Recommend next steps
Attachments
Add supporting documents:
- Assessment results
- Client photos (with consent)
- Supporting documentation
Importing Data
System auto-imports:
- Appointment history
- Session notes
- Goal progress from app
- Incident reports
Review and edit as needed.
Viewing Reports
Report List
- Go to Reports → DSHS Reports
- Filter by:
- Client
- Status
- Type
- Date range
Report Dashboard
See at a glance:
- Reports due soon
- Overdue reports
- Recently submitted
Due Dates
90-Day Cycles
Reports due every 90 days from:
- Initial plan date, OR
- Previous report end date
Notifications
System alerts:
- 14 days before: Upcoming
- 7 days before: Due soon
- On due date: Due today
- After: Overdue
Tips
- Start early — Don't wait until due date
- Import data — Use auto-imported content
- Review history — Include relevant events
- Check requirements — Meet state standards
