Create and track transportation requests.
What Is NEMT?
Non-Emergency Medical Transportation (NEMT) for clients needing rides to:
- Day programs
- Medical appointments
- Community activities
- Other authorized destinations
Enabling Transportation
- Go to Settings → Features
- Enable Transportation
- Transportation options appear on clients
Client Setup
Assign NEMT Broker
- Go to client profile → Transportation
- Enable NEMT Eligible
- Select Broker:
- Hopelink
- NWRC
- Access
- DART
Mobility Information
| Field | Options |
|---|---|
| Mobility Status | Ambulatory, Wheelchair, Walker, Cane |
| Wheelchair Type | Manual, Power |
| Special Needs | Service dog, lift van, extra time |
| Support Person | Rides with support person? |
Creating NEMT Request
Step 1: Start Request
- Go to client → Transportation tab
- Click + New NEMT Request
Step 2: Service Details
| Field | Description |
|---|---|
| Service Type | Community Access, Habilitative, etc. |
| Destination | Where they're going |
| Purpose | Why they need transport |
Step 3: Schedule
| Field | Description |
|---|---|
| Schedule Type | One-time or Recurring |
| Days | Which days (M, T, W, etc.) |
| Pickup Time | When to be picked up |
| Return Time | When to return |
Step 4: Case Manager
| Field | Description |
|---|---|
| Assessor Name | Case manager name |
| Assessor Phone | Case manager phone |
Step 5: Submit
Click Create Request → PDF generated
NEMT Request PDF
System generates formal request:
- Client information
- Service details
- Schedule
- Mobility needs
- Case manager contact
Sending to Broker
- Download PDF
- Fax or email to broker
- Broker contacts case manager for approval
Trip Occurrences
Each scheduled trip creates an occurrence:
Viewing Trips
- Go to NEMT request
- See Trip Occurrences
- Each date listed with status
Trip Status
| Status | Meaning |
|---|---|
| Pending | Awaiting confirmation |
| Confirmed | Broker confirmed |
| Completed | Trip happened |
| Cancelled | Won't happen |
| No Show | Client wasn't there |
Updating Trips
- Click trip occurrence
- Update:
- Transportation company
- Actual pickup time
- Status
- Save
Linking to Appointments
Trips auto-link to matching appointments:
- Same client
- Same date/time
- Staff sees transportation in app
Staff view shows:
- "Transportation scheduled"
- Pickup time
- Transportation company
Cancelling NEMT
Cancel Single Trip
- Open trip occurrence
- Click Cancel
- Select reason
- Confirm
Cancel Recurring Request
- Open NEMT request
- Click Cancel Request
- All future trips cancelled
- Past trips preserved
Reports
Transportation Schedule Export
- Go to Reports → Transportation
- Select facility or all
- Select week
- Export Excel with:
- Pickup times
- Client names
- Transport company
- Destinations
Tips
- Fill out completely — Brokers need all details
- Update trips promptly — Track what happened
- Link to appointments — Staff stays informed
- Export weekly — Share with facilities
