Who Is Your Care Team

Your Team Includes

  • Staff who visit you regularly

Viewing Your Team

From the Portal

  1. Log in to the portal
  2. Go to Care Team or My Team
  3. See team members listed

Information Available

For each team member:

  • Name
  • Role
  • Photo (if shared)
  • How they support you

Scheduled Staff

Who's Coming

Your schedule shows:

  • Which staff is assigned
  • For each appointment
  • Their expected arrival

Regular Staff

You may have:

Communication

To communicate with staff:
